Introduction: Acute appendicitis is considered an acute medical condition needing a rapid and accurate diagnosis. Laboratory and images are used to confirm the diagnosis but, at the end biopsy is confirmatory. Not all medical facilities have images studies and a scale with signs and symptoms can support a clinical suspicious for diagnosis. Objective: To compare the sensitivity and specificity of the Alvarado Scale for the diagnosis of acute appendicitis confirmed by histopathological examination This is a prospective, retrospective, observational, and cross-sectional study. Methodology: The studied sample consisted of 219 patients with an acute abdomen diagnosis suggestive of acute appendicitis. All patients underwent either an open appendectomy or a video laparoscopy. They were clinically evaluated under the Alvarado Scale. The diagnostic value of the scale alone was established and compared to the gold standard, which consists of the histopathological examination. Results: Out of the 219 patients included in the study, 178 were diagnosed using the Alvarado Scale, which showed a specificity of 53% and sensibility of 89%, as well as a positive predictive value (VP) of 88%, and a negative VP of 53%.
