Comparación Clínica y Radiológica de Osteoartritis de mano erosiva vrs Osteoartritis de mano en un centro reumatológico guatemalteco

Resumen

Introducción: Crain, en 1961, identificó presencia de inflamación en pacientes con osteoartritis de mano utilizando para ello el término de osteoartritis inflamatoria, cinco años después Peter et al. Observó alteraciones radiográficas erosivas en algunos casos de osteoartritis de mano. Objetivos: identificar la prevalencia de Osteoartritis erosiva de mano (EHOA), sus factores de riesgo y la sensibilidad de los criterios propuestos por Anandarajah. Metodología: Descriptivo de corte transversal, se comparó 400 pacientes 22 EHOA Y 378 Osteoartritis de manos (HOA) de enero 2021 a febrero 2022 recolectada por revisión de expedientes, los criterios de Anandarajah fueron utilizados.

Resultados: La prevalencia de EHOA es de 5.82% por sexo varía del 4.76 al 1.06% en mujeres y hombres respectivamente, su inicio fue abrupto, el principal factor de riesgo es obesidad con diferencia estadísticamente significativa entre los grupos estudiados, otros factores son sexo femenino, hipertensión, dislipidemia, diabetes mellitus, trabajo manual y trauma articular con valores individuales mas elevados comparados con la HOA. Los criterios diagnósticos estudiados muestran una sensibilidad del 65%

Conclusiones: EHOA presentó mayor componente inflamatorio, inicio abrupto de los síntomas y asociación a factores de riesgo que le predisponen a un estado proinflamatorio crónico; consideramos que la EHOA podría tener una etiopatogenia distinta que la diferencia de la HOA, empero estudios histológicos grandes serian necesarios.
